FILLING OUT THE ATF FORM 1 - PAGE 1
I have received a few requests to provide some instruction on how to file out this form and get it right the first time so ATF does not kick it back to you for corrections.  It is not a 100% guarantee mind you (since I do not work for the NFA) but I suppose it is better than nothing!

For individual LE officers and private citizens alike, if you are going to SBR your personal AR-15 or other rifle, you need to fill this form out and in addition get two fingerprint applicant cards that read on the ORI "DCATF Firearms & Explosives, Washington, DC."  Do not get fingerprint cards from your local ATF office as it will appear that you are applying for a position with the ATF instead.

If you are a LE agency and are building a post-1986 machine gun for duty use you also use this form.

Let's go box by box and walk you through it:

1. Type of Application:
If this is a department-owned firearm you would file the Form 1 but check the box either "b" or "c" that says this is tax exempt because it is being made for a government entity.  Check "b" if it is for a federal agency, check "c" if it is any other government agency other than federal LE.  All privately owned weapons check box "a".

2. Application is made by: check "individual" if it is a privately owned firearm.  Check "corporation or other business entity" if you are making it as a corporate asset and are bypassing the CLEO signature.  Check "government entity" if it is a department-owned weapon.

3a. Trade Name:
if you are an FFL (not a Class II/Type 07) write your FFL trade name in here, or if you are a Corporation write the Corporation name here.  Otherwise leave it blank.

3b. Applicant's name/address
: fill out your legal name and residence address.

3c. If you listed a P.O. box in 3b, write your legal residence address here.

3d. County:
list the county that your legal residence is located in.

3e. Telephone Number: Write your telephone number in the event ATF needs to contact you.  Cellular phone is sufficient.

4. Description of Firearm:

a) Original Manufacturer: Write down name and address of original manufacturer that is stamped on your receiver, such as:

Bushmaster Firearms
Windham, Maine

b) Type of Firearm: Select the one you want to make.  Note it does not have a "machine gun" selection, in that event just delete the field and make it blank, and fill it out after you print it.  (post-86 machine guns only).

c) Caliber/Gauge:
make sure it is complete: 12 ga, .223 cal., 5.56 mm, etc.

d) Model:
List the model the manufacturer designated for this weapon originally UNLESS it is a Colt AR-15 rifle.  If it is a Colt rifle list it as "AR-15," not "LE6920", "Law Enforcement Carbine," etc.  Regardless of what Colt stamped on the receiver, ATF has kicked these back before for not having "AR-15" written here.
