4e) Barrel Length: list the barrel length in inches.
f) Overall Length: list the short possible configuration, i.e. with the stock closed.
g) Serial Number: If you are using an existing firearm then use the original serial number. If you are making your own weapon from scratch then you would write in the serial number you have engraved into it.
h) Additional Information: If you had a Colt AR-15 style rifle you can list here the actual model number that is stamped on the receiver such as "Colt's Law Enforcement Carbine."
i) State Why You Intend To Make Firearm: if you are going to use it on duty you can write "Law Enforcement Duties." Most citizens write "for collecting purposes."
5) FFL #: if you are an FFL that is not a Type 07 and a Class II SOT then list your FFL number. Otherwise just write in "N/A"
6a and 6b) EIN and Class: again if you are not an FFL write in N/A, if you are a FFL/SOT not a C2 Manufacturer then write your information in.
7a - 8e): These should all be "no." If you have a "yes" you will be automatically disqualified.
9) Signature of Applicant: sign your legal signature here.
10) Name/Title of Authorized Official of Firm or Corporation: Print your name and title. If no title, just write "Owner." Example: "John Henry Smith, Owner."
11) Date: Write in the date you signed the form.
12) Photograph: Affix your passport photo here. I recommend using rubber cement.
13) Law Enforcement Certification: Write your name where it says "name of maker." Your LE agency should fill out the rest. If they only sign it, make sure you write in their information (Organization Name, Address, and Date).
Print this page out twice, but make sure both copies are double sided (Page 1 on front, Page 2 on back). You will also need:
2 Passport Photographs
2 Applicant Fingerprint Cards (ORI must state DCATF Firearms & Explosives Division, Washington, DC.)
-in the box that asks the reason why you are being fingerprinted write in "NFA Applicant."
1 Certification of Compliance Form 5530-20. You can download it below.
-If you are U.S. Citizen you would write your name in (last, first, middle) on both lines 1 and 2.
-"United States of America" is already filled in Line 3. Skip to line 7 and sign/date this form.
1 personal check payable to ATF for $200. That way you know when they cash the check which means it has been assigned to an examiner. Now you can start waiting. Even if you build an AOW it is still $200 since you are not a C2 SOT.
